Fixed critical bug where no notifications were sent for users
Now all users should start getting notified when their favourite swimmers are added to a startlist, or when they have gotten new results.

New Features
New views have been added in Swimmers view.
It is now possible to see all entries per club and all heats per club, either grouped by event or by swimmer.

* Added possibility to show combined result lists from competition series and combining the results from multiple competitions into one list. Can be used for example in series of competitions with qualifying meets for championship meets, where multiple competitions are run simultaneously in different locations.

Bug fixes:
* Fix star in header for swimmer details and lane details to be correct
* Fix bug where club logo not always where shown in favourite swimmer details list
* Fix bug with splits for relay teams, 100m and 200m splits are now shown correctly for relay teams and not just for the first swimmer.
* Fix list of last viewed competitions to show the 6 last viewed competitions, instead of just one competition.
